NOTICE REGARDING ELECTRONIC RECEIPT OF FUND DOCUMENTS
The United Association National Pension Fund ("Fund") offers electronic retrieval of the Annual Benefit Statement ("Statement") through individual, secure log-in on the Fund's website. This notice requests your affirmative consent to receive the Statement through this means. If you do not consent to obtain the Statement through the website, the Fund will mail a copy of the Statement via U.S. Mail. The Fund strongly encourages (but does not require) Participants to obtain the Statement electronically, as it assists the Fund to conserve assets for the payment of benefits.
In order to obtain the Statement through the website, you must have access to a computer or other electronic device capable of connecting to the Internet. You must also have a valid e-mail address to which the notice that you have a new Statement can be sent. It is important that you notify the Fund of any change to your e-mail address. If your e-mail address has changed, please use the Edit Your Account Information link to provide your new e-mail address. You have the right to obtain a paper copy of your Annual Benefit Statement, which will be provided free of charge. You may request a paper copy by calling the Fund office at 1-800-638-7442 or on-line at Contact Us . Your Statement will be on the website as a PDF file, which you can print or save to your computer. You may withdraw your consent to obtain the Statement electronically at any time without charge. To withdraw your consent, change your preferences through the log-in on the website, or send an e-mail or letter to the Fund stating that you are withdrawing your consent to obtain the Annual Benefit Statement through the website. If you consent to obtain the Annual Benefit Statement electronically via log-in on the Fund's website, check the box below to indicate your consent. Please contact the Fund Office with any questions.
